Registering with the Practice

Non-urgent advice: Registration

Please Note:
We are unable accept new patients who are already registered with a local practice.

If you are applying to register with our practice using our online registration form, please ensure that you read this carefully and complete all sections that apply to you. Failure to complete the form will result in delays to your registration.

To register:

  • fill out an online registration form
  • visit reception during surgery hours and collect a paper registration form
  • we ask that you provide proof of you address when registering. Photo ID is not required.
  • the practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

If you do not stay within our catchment area other GP practices can be found by accessing the button below.

Registered with a Doctor outside the UK

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.

Step 1: Download the Forms

You can download then complete the registration forms, and then upload them to the practice on the button below.

 If you are unable to do this please contact the surgery for advice on how to register.

Please note: we do not automatically continue prescriptions of Methadone, Tramadol, Benzodiazepines, Gabapentine, Pregabalin, Dihydrocodeine and other Opiates.

Once you have completed the forms please upload the forms to the practice. 

Step 2: Upload proof of address

Please upload proof of your address – this must be a permanent address and within our practice boundary (see above).
